All that is required is yourself, although it is useful to bring the following with you:
- GP referral letter
- Previous neurology letters
- Previous medical history with relation to your complaint
- Prescription with current medicines (or the medicines themselves)
Please note you are more than welcome to bring along a family member or a friend to your appointment.
